Abstract #756: Impact of Infliximab on Costs of Ankylosing Spondylitis: Results of Eres Study, and Economic Evaluation Based on One-Year Randomized Controlled Trial in a One-Year Open-Label Extension Study

Purpose: The purpose of this study is to estimate the reduction in both direct and indirect healthcare costs as a result of patients with active AS of using Infliximab. 

Methods:  This study reviewed 247 patients with AS and active disease (BASDAI = 6.2).  These patients were receiving Infliximab by two different administration strategies.  One group was receiving infusions (5mg/kg) every six weeks, the other group was receiving infusions (5mg/kg) “on demand.”  Patients were surveyed every three months through an entire year to report their sick leave taken as well as healthcare obtained.  This was compared to the sick leave and medical costs in these same patients six months prior to initiating therapy.

Results: In both groups, there were significant and equally reduced direct and indirect costs.  Specifically, 48% reduced medical visit costs, 28% reduction in physical therapy and rehabilitation costs, and 26% reduction of inpatient care costs.  Also, when looking at those patients that were in the workforce, there was a 40% reduction in sick leave.

Comment: These analyses are extremely important as we look not only at the clinical effect but also the economic effect of TNF inhibition.  This demonstrates benefit not only to the patient, but also to society.  In going forward with similar studies, it will be important to evaluate these outcomes over longer periods of time, especially as lower cost strategies are considered (i.e., decreased dosage, less frequent infusions).
